A total of 453 consecutive patients who underwent open laparotomy with gastroenterological surgery, performed by a single surgeon at a single hospital, were investigated between August 1, 2011, and August 31, 2022. A consideration of 23-pentanedione and its analogues, like acetoin (a reduced form of diacetyl), is warranted, especially given the insights gained from recent toxicological studies. Data from the current work relating to the mechanistic, metabolic, and toxicological aspects of -diketones were the focus. For diacetyl and 23-pentanedione, a comparative pulmonary impact assessment was undertaken leveraging the most abundant data. This resulted in a proposal for an occupational exposure limit (OEL) for 23-pentanedione. Previous OELs were subject to a review, and a new literature search was undertaken.
